I am happy to give the personal details and information required for processing this form. I understand that they will be valid and stored for 2 years from the time that this form is submitted, at which point I will need to provide consent again for continued use of the photo, film or words. I acknowledge that Kirklees Council may contact me to request continued consent. I understand that by giving written consent for the use of the photo, video/voice recording, name or quote it could be used in Kirklees Council publications and may be shared with other organisations. I understand that I can contact Kirklees Council at any point and ask them not to use the photograph, film or words for any new purpose. I am aware that once an article is published and in circulation (especially online) it may be copied and used by others. I understand that if I ask Kirklees Council not to use the photograph, film and words in the future they will comply with my request and will do their best to stop others doing so, but they cannot guarantee this.
